HUM 102 Fundamentals of Case Management

Case management is the basis of effective health and human service practice. In this course you will learn the basics of case management including client assessment, collaborative problem solving, treatment planning, referral, participation in treatment teams, record keeping and outcomes evaluation. You will consider the role of case manager as the “hub” of treatment planning, referral, and evaluation, as well as learning about how the role of case manager is vital to effective cost containment, high quality care, and consumer satisfaction.

Learning Outcomes
  1. Define and apply case management as a method of service delivery across a variety of sectors.
  2. Describe case management core competencies and their applications in the day-to-day work of case managers.
  3. Characterize key concepts of philosophy, values definitions and case management models that underpin the practice of case management.
  4. Discuss the implications of ecosystems theory as it pertains to the client system and the environment in which intervention, assessment, and services take place.
  5. Identify the structure of the formal community service system and the informal support system, and explain their importance for case managers.
